Terms of Service

These Terms of Service ("Terms") govern your use of the website located at www.glcwellness.com ("Site") and the mental and behavioral health services offered by Golden Life (GLC) Wellness Center, LLC and its affiliated clinics ("GLC," "we," "our," or "us"). By accessing the Site or scheduling services with us, you agree to be bound by these Terms.

Not for Emergencies

GLC services are not designed for emergencies. If you are experiencing a mental health emergency or crisis, call or text 988 (Suicide & Crisis Lifeline), call 911, or go to your nearest emergency room.

1. Acceptance of Terms

By using the Site, scheduling an appointment, or receiving services, you confirm that you have read, understood, and agree to these Terms and our Privacy Policy & HIPAA Notice of Privacy Practices. If you do not agree, please do not use the Site or our services.

2. Description of Services

GLC provides outpatient mental and behavioral health services, including individual therapy, couples and family therapy, group therapy, psychiatric evaluations, medication management by Psychiatric Mental Health Nurse Practitioners (PMHNPs), and treatment planning. Services are delivered in person at our clinic locations and via telehealth, where permitted by applicable law.

4. Telehealth Disclosure

Telehealth services are delivered using HIPAA-compliant platforms. Telehealth carries inherent risks, including but not limited to: technology failures, delays in care due to connectivity issues, inability to perform physical examinations, and the small risk of unintended disclosure despite reasonable safeguards. Telehealth may not be appropriate for all conditions, and your clinician will determine, in their professional judgment, whether telehealth is suitable for your care. Telehealth services are provided only to clients who are physically present in a state where the assigned clinician is licensed at the time of the session.

5. Eligibility

You must be at least 18 years of age to schedule services on your own behalf. Services for individuals under 18 must be arranged by a parent or legal guardian who consents to treatment. We may require documentation of guardianship or custody before providing services to a minor.

6. Appointments, Cancellation & No-Show Policy

You may request, reschedule, or cancel appointments through our client portal (SimplePractice) or by contacting our office. To avoid a cancellation fee, please cancel or reschedule at least 24 hours in advance of your scheduled appointment. Late cancellations and no-shows may be subject to a fee, which will be disclosed at the time of intake. Repeated no-shows may result in discharge from our practice.

7. Fees, Billing, and Insurance

Fees for services are disclosed at intake and may vary by service type, clinician, and location. We may participate with select insurance plans and accept private pay. You are responsible for verifying your insurance benefits and for any deductibles, copayments, or amounts not covered by insurance. Unpaid balances may be subject to collection. Detailed financial policies are provided at the time of intake.

Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges for non-emergency services. We will provide a Good Faith Estimate upon request, or automatically when required by law.

8. Client Portal and Electronic Communication

Our client portal is operated by SimplePractice and protected by HIPAA-compliant safeguards. You are responsible for maintaining the confidentiality of your portal credentials and for all activity that occurs under your account. Standard text messages and emails are not encrypted; do not transmit clinical or sensitive information through unencrypted channels. Use the secure portal for clinical communications.

9. Intellectual Property

All content on the Site, including text, graphics, logos (including "GLC Wellness Center," our bird mark, and the tagline "Giving. Life. Clarity."), images, audio, video, and code, is owned by or licensed to GLC and is protected by copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our prior written permission, except for personal, non-commercial use.

10. Acceptable Use

You agree not to:

  • Use the Site for any unlawful purpose or in violation of these Terms.
  • Attempt to gain unauthorized access to any portion of the Site, our systems, or the client portal.
  • Interfere with the operation of the Site, including through introduction of malware, viruses, or excessive automated requests.
  • Impersonate any person or misrepresent your affiliation with any person or entity.
  • Submit false, misleading, or fraudulent information.
  • Harass, threaten, or harm our staff, clinicians, or other clients.

11. Third-Party Links and Services

The Site may contain links to third-party websites (e.g., Google Maps, social media platforms, SimplePractice). GLC is not responsible for the content, accuracy, or privacy practices of third-party sites. Your interactions with third-party services are governed by their respective terms and privacy policies.

12. Disclaimer of Warranties

The Site and its content are provided on an "as is" and "as available" basis without warranties of any kind, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, non-infringement, or accuracy. We do not warrant that the Site will be uninterrupted, error-free, secure, or free of harmful components. Information on the Site is for general informational purposes only and is not medical or clinical advice. Always consult a qualified clinician for personal health concerns.

13. Limitation of Liability

To the fullest extent permitted by applicable law, GLC, its officers, directors, employees, contractors, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, data, use, or goodwill, arising out of or related to your use of the Site, even if advised of the possibility of such damages. Our total aggregate liability for any claim arising from your use of the Site shall not exceed one hundred dollars (US $100). This limitation does not apply to liability that cannot be limited under applicable law, including liability arising from the direct provision of clinical services, which is governed by applicable professional licensing and tort laws.

14. Indemnification

You agree to indemnify and hold harmless GLC and its officers, directors, employees, and agents from any claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or related to: (a) your breach of these Terms; (b) your violation of any law or the rights of any third party; or (c) your misuse of the Site.

15. Governing Law & Venue

These Terms are governed by the laws of the state in which the GLC location providing your services is situated, without regard to its conflict-of-laws principles. Disputes arising from services at a particular location are governed by the law of that state. Disputes arising solely from use of the Site (without clinical services) are governed by the laws of the State of Illinois. Venue for any legal action arising from clinical services shall lie in the state and county of the relevant GLC location. Venue for Site-only disputes shall lie in Cook County, Illinois.

16. Dispute Resolution

Before initiating any legal action, you agree to first contact us in writing at info@glcwellness.com to attempt to resolve the dispute informally. If the dispute is not resolved within thirty (30) days, either party may pursue available legal remedies. Nothing in these Terms prevents you from filing a complaint with the appropriate state licensing board or the U.S. Department of Health and Human Services.

17. Changes to These Terms

We may update these Terms from time to time. The current version will always be posted on this page with an updated "Last Updated" date. Continued use of the Site or services after changes constitutes acceptance of the revised Terms. Material changes affecting your treatment relationship will be communicated to you directly.

18. Severability

If any provision of these Terms is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

19. Entire Agreement

These Terms, together with our Privacy Policy & HIPAA Notice of Privacy Practices and any signed client agreements, constitute the entire agreement between you and GLC regarding the subject matter herein.
